About Willow Care Navigation
​Willow Care Navigation is a private, RN-led care navigation service supporting patients and families during complex medical transitions.​ After hospital stays or short-term rehab, families are often left to coordinate medications, appointments, services, and next steps with little guidance. Willow Care Navigation exists to bring clarity, organization, and compassionate nurse support during these critical moments.
​My role is to help you understand next steps, review medications and care instructions, prepare for follow-up visits, and communicate more effectively with healthcare providers. I support families in knowing what to monitor at home, what information providers may be looking for, and how to feel more confident and prepared as care continues.

Hi, I’m Carla, a Registered Nurse with experience caring for patients across hospital and community settings.

I created Willow Care Navigation because I saw firsthand how overwhelming the healthcare system can be for patients and families — especially during transitions between levels of care. Too often, people are discharged with stacks of paperwork, unanswered questions, and no clear plan.
​
I believe families deserve personalized, nurse-led guidance that is practical, compassionate, and focused on real-life needs.

My approach is calm, organized, and centered on advocacy, education, and respect for each person’s goals.​
